This statement discloses the privacy practices and policies for www.pkc.com, the PKC corporate Web site.

Types of Information Collected

To make use of certain features on our Web site (forms, surveys, for example), you must provide certain information such as an e-mail address.

Feedback Form

The Feedback Form is for users to submit suggestions to PKC regarding Coupler content, usability, and functionality. You are solely responsible for the content of your submission. Do not submit proprietary, confidential, or personal information. Any submissions containing such information will be subject to deletion by PKC because we will not keep personal health information.

In your feedback, avoid using personal pronouns such as I, me, and my. Instead, use generic third-person terms such as patient or user. For example, "The patient was confused by the questions about medication use."

The following may happen to the information you provide on the Feedback Form:

  • It will be sent to PKC via e-mail.
  • It may be saved in a PKC-controlled database.
  • It may be shared with PKC employees, including Coupler builders and software engineers.
  • It may be used to contact you for discussion or clarification regarding your submission. PKC will not contact you for any other purpose. If you do not wish to be contacted regarding your submission, please say so in the last response field of the form.
  • It may be used by PKC to guide further development of Couplers.

Information that might personally identify you will not be distributed outside PKC without your permission or unless required by law. However, if you note that you want to be acknowledged for your feedback, you are granting permission for PKC to publicly identify you in recognition of your contribution.

Request a Demonstration Form

The Request Form is one way that you can ask for a demonstration of Couplers.

The following may happen to the information you provide on the Request Form:

  • It will be sent to PKC via e-mail.
  • It will be used to contact you to arrange a demonstration.
  • It may be saved in a PKC-controlled database.
  • It will be shared with PKC employees.

Information that might personally identify you will not be distributed outside PKC without your permission or unless required by law.

Purchase Couplers Form

The Purchase Couplers Form is for healthcare consumers to subscribe to Couplers for the Web.

The security of your personal information is important to us. PKC uses PayPal™, a respected third party, to process credit card payments. The credit card information you enter on this form is submitted to PayPal using secure socket layer technology (SSL). PKC does not keep a copy of this information and it is not available to PKC employees. Your credit card will be used to conduct the following transactions:

  • The initial purchase.
  • Recurring subscription payments.
  • Refunds (when applicable).

The following may happen to all other information you provide on the Purchase Couplers Form:

  • The username, password, and subscription details will be sent to the e-mail address you provided.
  • It will be saved in a PKC-controlled database and a limited number of PKC employees will have access to it.
  • It may be used by PKC to contact you regarding your subscription.
  • It may be used by PKC to provide customer service to you.

Information that might personally identify you will not be distributed outside PKC without your permission or unless required by law.

If your personal subscriber information changes, or if you no longer desire our service, you may correct, update, or deactivate it by e-mailing our Customer Support at support@pkc.com, or writing PKC at our corporate offices at 1 Mill Street, Box C13, Burlington, Vermont 05401.

Preview a Coupler

PKC periodically offers complimentary access to one or more Couplers to let you get a sense of the depth and breadth of the medical knowledge provided in Couplers. Once you connect to any Coupler, you have left the PKC corporate Web site and a separate privacy policy applies.
